As you may be aware, there are two different Lasting Powers of Attorney which you can put in place. One relates to property and financial affairs and the other to personal welfare.
This questionnaire highlights the points on which we require your clear instructions, before drafting your Lasting Powers of Attorney, for example:

- Your choice of replacement Attorneys (if any)
- Persons to be notified when the Power is registered
- Who is to be appointed as the Certificate Provider
